A Walk to Care network consists of Contract Volunteers and Contract-for-fee Mental Health Providers. Mental Health Providers include but are not limited to Clinical Social Workers, Certified Counselors, Marital Counselors, Pastoral Counselors, Psychologists, Psychiatrists, and Nurse Psychiatric Practitioners.

Please send us your information and a Provider Specialist will assist you with completing a Provider Application and Agreement. Providers will be required to submit State Licensure or Registration with an Expiration Date, Resume, W9 Tax ID Form, and Minimum Liability Insurance Coverage of $1 million/$3 million along with a Provider Application and Agreement.
